Home Care Partners (www.homecarepartnersofnebraska.com) celebrates the continued education of Managing Partner Karla Frese. Karla completed the Nebraska Healthcare Association comprehensive educational course that provides training to understand regulations, policies, and best practices for social services in healthcare settings. The coursework included reviewing admission processes, creating individualized care plans, and discharge planning. The course allows one to better understand the social service role and how to coordinate care services with interdisciplinary healthcare teams.
“I value each day that I get the opportunity to make a difference in someone’s life,” said Karla. “Helping others gives me a sense of belonging, and can change your life in ways you never imagined.”
As a social services designee working for Home Care Partners, Karla has the chance to work with healthcare teams and facilities to make a positive impact in the lives of Home Care Partners’ clients. When someone has a health change in their life she can provide guidance and understanding when they need to navigate what comes next in
the continuum of care.
